Extraction of bicuspids for orthodontic treatment

The dental treatment that disturbs me the most is the serial extraction of bicuspids for orthodontics. Decades ago a Dr. William Proffit proposed a treatment regimen to correct crowding by removing (2) or (4) bicuspids. In 2019 there still exist many clinicians who utilize the archaic treatment regimen. Recently a colleague sent me models of a 12 year old who was in the midst of orthodontic treatment where she had her bicuspids removed. Horrendous facial aesthetics and poor jaw position are the result of this illogical treatment regimen. Our practice endorses orthodontic/orthopedic treatment where we recognize crowding is a function of a skeletal deficiency,
not a product of too many teeth. Our patients end up with big beautiful smiles and healthy TMJ’s.
